Discharge due tomorrow - and then?

Many a patient, as well as his or her relatives, may be faced with this question if their lives have been changed irrevocably by an accident or illness. Together with those affected, the social service at the diaconal hospital wants to find solutions and open up perspectives, for example:

• initiate a prescribed rehabilitation or the therapy
  
• organise care at home or at a nursing home

• answer questions about health insurance, pension schemes, National Insurance and health care insurance, or
  make contact with the respective offices.
